This notice describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.

Our Commitment

Insightful Psychiatry is required by the Health Insurance Portability and Accountability Act ("HIPAA") to protect the privacy of your Protected Health Information ("PHI"), provide you with this Notice, and follow its terms.

How We May Use and Disclose Your PHI

  • Treatment — to provide and coordinate your psychiatric care.
  • Payment — to bill and collect for services rendered.
  • Health Care Operations — quality improvement, training, and administration.
  • As Required by Law — public health, abuse reporting, court orders, and other legally required disclosures.

Uses Requiring Your Written Authorization

We will obtain your written authorization before using or disclosing PHI for marketing, before selling PHI, or before disclosing psychotherapy notes (except as permitted by law). You may revoke an authorization at any time in writing.

Your Rights

  • Request access to and a copy of your PHI.
  • Request an amendment to your record.
  • Request an accounting of certain disclosures.
  • Request confidential communications or restrictions on certain uses.
  • Receive notification of a breach of unsecured PHI.
  • Receive a paper copy of this Notice upon request.

Complaints

You may file a complaint with us or with the U.S. Department of Health and Human Services, Office for Civil Rights. We will not retaliate against you for filing a complaint.
